"Dry socket" occurs when the clot either doesn't form or is removed before all healing has taken place..and actually is any socket in which a patient is having pain due to the loss of the blood clot thus exposing the bone to air, food, and fluids along with an offensive odor. This often occurs two or more days after an extraction and can last about 5-6 days. It is normal to have soreness and discomfort following an extraction. Pain should be lessening by the second day. When
the blood clot is lost before the underlying structres have had time
to heal, bone is exposed to the oral environment along with fine
nerve endings. Its similar to when kids will "pick at a scab" and
infection occurs because bacteria can now get into an open wound.
There are some activities which may increase the propensity for dry socket formation...smoking, drinking carbonated beverages in the first 24 hours after surgery, spitting or drinking through a straw in that same time period...but often "dry sockets" occur for no particular reason at all. If a patient of mine is having an extraction, I have them use the tinctures of Calendula, Hypericum (St. John's Wort) and Goldenseal. Get the small gauze squares fold it once and drizzle several drops of each tincture on the gauze and bite down gently on it over the opening. Calendula is a wonderful herbal hemostat and anti-bacterial, Hypericum really tones down the pain and Goldenseal is also a great non-infective agent.
