CONSULTATIONS
If
you would like your health history, or that
of your pet's reviewed personally by me, followed by a personal
consultation, and an evaluation of your bio-field with the e-Lybra
please fill in the forms below and e-mail back to me at Dr.
Deb's E-Mail or fax to 647-342-8186. At the end of the questionnaires,
please write a personal note about yourself/pet including the following
Date
of birth (very important)
Last
time you were well
List
all medications or hospital stays
Any
tests that you have had and the results.
What
are your symptoms today, the things that really get in the way of
you living the life you want
Childhood
health history.
All
vitamin/mineral supplements and any herbal/homeopathic remedies you
are currently taking.
Include
your e-mail address and phone number
Then,
I will need a hair sample, about the amount that
would fit on a 50 cent piece. It should be from the nape of
the nect and if you colour your hair, take the sample just before
you are going to re-colour, so I have as much of your natural colour
as possible. If this is for your pet..I need the same size sample
of fur or feathers.
Place
this in a clean envelope, fold it into another envelope and send to:

Fee:
$350.00 (U.S. dollars) payable by PayPal, Visa
or MasterCard
The
test on the e-Lybra takes me over an hour to run and the phone consultation
will last 1/2 hour to an hour..what ever we need.
If
you are already a patient and would like to have an analysis by e-Lybra
8 the fee is the same as a subsequent consultation, $175.00 U.S.,
billed through our office or HERE You would need to send the
hair sample and date of birth to the address above.
We
process the consultation fee throught the office
or the store so credit card info can be e-mailed
or you may call toll free from the US and Canada 1-877-402-5450
Once
I have received this information, you can them contact us by e-mail
or the toll free number to set up a consultation time. I will discuss
all apspects of your health during our consultation (usually lasting
somewhere between one half to one hour and then I will set up a protocol
for you and we can supply your supplements or you can take it to your
holistic health care practitioner and blend it with their suggestions.
